Med-legal narrative reports: the workflow for orthopedic groups

Facts as of October 19, 2026. Operations and software only; not clinical, coding or legal advice.

A narrative report is a physician’s written account of an examination, the records reviewed and the physician’s opinions, prepared for an insurer, an employer or a court. For an orthopedic group that does med-legal work, the operational problem is turning a pile of records into a reviewed, signed document on time, again and again.

This page is about paperwork and process. It is not legal advice, and case decisions belong with counsel.

The documents in the workflow

The pipeline, step by step

  1. Intake. The request arrives with a due date and a requester.
  2. Records. Staff collect records, and note what is missing.
  3. Review. Someone reads the records and flags the facts the report will rely on, each with its source page.
  4. Draft. The report is drafted from those facts.
  5. Physician review and signature. The physician checks the draft against the records and signs.
  6. Delivery. The final report goes to the requester, with a log of when.
  7. Tracking. Someone watches due dates and follow-ups across every open case.

Work usually stalls in the gaps: records that arrive late, a draft waiting for a physician’s attention, a due date held in one person’s calendar. A tool that keeps one record per case, with an owner and a due date, makes those waits visible.

Each program has its own forms, deadlines and rules, and they change. Use the agency pages below as the source of truth and check the current version before you rely on a form.
